Please review, print out, and sign (if possible) the Chromebook Insurance Program and Computer Needs Agreement Form before arriving at PGJH on your scheduled Chromebook pickup night. You only need print off Page 5 (the last page). If you lose your copy or are unable to print off a copy we will have extras at PGJH, but to speed up the checkout process having this form already signed would be much appreciated.
Please bring $30 per student or $60 per PGSD family on the night of your pickup. Checks may be made out to Prairie Grove School District.
