NEW STUDENT REGISTRATION
Registering your child at Prairie Grove now consists of two steps:
Registration: Using our secure web portal the parent or guardian will enter the student’s information and complete the required registration forms.
NOTE: You will need to have all required documents available when you register your child.

The following documents are REQUIRED DOCUMENTS. You will be directed to upload these documents during the online enrollment process
PROOF OF RESIDENCY
A current utility bill (within 30 days) showing the name and address of the student’s and parent’s home.
A home purchase contract including a specific closing date with a copy of the deed to be provided within 90 days.
Mortgage or Rental Contract
An approved School Choice application
Documentation of an approved legal transfer between school districts.
Families living in a situation where these documents are in another person’s name will need to complete the Address Verification Form (Click Here)
If you answer Yes to the question in the registration “Is your child homeless?”, then please complete the McKinney-Vento form (Click Here)

SOCIAL SECURITY CARD

This is required for Preschool and Kindergarten students only.

IF YOU ARE NOT ABLE TO UPLOAD THESE DOCUMENTS, YOU WILL NEED TO PROVIDE THESE TO THE DISTRICT REGISTRAR which is located in the Prairie Grove School Administration office at 102 E. Buchanan Street. Please do not leave these documents are the school building’s offices.
REGISTRATION IS NOT COMPLETE UNTIL ALL DOCUMENTS HAVE BEEN PROVIDED AND ENROLLMENT HAS BEEN APPROVED BY THE DISTRICT REGISTRAR.
