Parents/Guardians:
The Prairie Grove School District will be holding a Mass Flu Immunization Clinic on October 25th. The Centers for Disease Control recommend an influenza (flu) vaccine every year as the first and best way to protect against getting the flu.
The Elementary School clinic will be held the morning of October 25th. The Middle School and High School clinic will be held the afternoon of October 25th. These clinics are for students and staff only. No parents are allowed to be present with their child per Washington County Health Department request. Instructions regarding the paperwork are attached to the permission slip form; please review this, and if you have questions, please contact one of the school nurses. You may also register your child at https://ADHSchoolFluPreReg.Arkansas.gov/. If you choose to register your child online you must remember to sign and submit a Prairie Grove School District FERPA form to your building nurse as well. Both forms are required for your child to receive the flu shot. Deadline to return a form is October 23rd.
